Before completing this application make sure you have as much information to hand as possible such as National Insurance number, names and details of anyone who will be giving you a reference, exam results and email addresses.

An Education, Health and Care plan ("EHC plan") is a legal document issued by the Local Authority which describes a child or young person's special educational needs, the support they need, and the outcomes they would like to achieve. Please submit a copy of your EHCP to: m.bardsley@thenhc.co.uk
